3. Document as required for filling the Application :

आवेदन भरने के लिए आवश्यक दस्तावेज़:

a. Photo and Pancard applicant

आवेदक की फोटो और पैन कार्ड

b. Income Certificate(Refer to Booklet for Income norms)

आय प्रमाणपत्र (आय मानकों के लिए बुकलेट का संदर्भ दें)

c. Reservation category Certificate (if Applicable).

आरक्षण वर्ग प्रमाणपत्र (यदि लागू हो)

d. Affidavit as per prescribed format in Booklet

बुकलेट में निर्धारित प्रारूप के अनुसार घोषणापत्र

e. Canceled Cheque

4. Please retain the original Affidavit. It will be required during allotment.

कृपया मूल घोषणापत्र को संरक्षित रखें। यह आवंटन के समय आवश्यक होगा।

5. Applicant is requested to give his own bank account details. Same will be used for refund in case of non allotment . any discrepancy in Bank details may lead to refund being initiated in wrong account. neither bank nor UPAVP will be responsible for any such act of omission by Applicant.

आवेदक से अपना बैंक खाता विवरण प्रदान करने का अनुरोध है। असमान्य आवंटन की स्थिति में धन वापस करने के लिए यही खाता उपयोग किया जाएगा। बैंक या UPAVP दोनों किसी ऐसे कृत्य के लिए जिम्मेदार नहीं होंगे, जिसमें आवेदक की कोई कमी हो।

6. Registration Cost is Rs. 1000 + GST. This is non Refundable .

7. Formats for uploading document :

दस्तावेज़ अपलोड करने के लिए प्रारूप:

a. Photo :Max 1 MB , (.jpg Format).

b. Other attachments : Max 1 MB each , (.pdf Format).
