I hereby declare that all the information furnished in this application form is true, complete, and correct to the best of my knowledge and belief. I understand that if any information provided is found to be false or misleading, my empanelment is liable to be cancelled without any notice.

I undertake to comply with all rules, regulations, and directions issued by the UP Housing & Development Board (UPHDB) from time to time. I shall not act in any manner that is detrimental to the interests of UPHDB or its allottees/customers.

I give my consent to the UP Housing & Development Board (UPHDB) or any authorized agency to verify the documents and details submitted by me for the purpose of empanelment. I also authorize UPHDB to conduct background checks as deemed necessary.

I hereby declare that neither I nor my firm has been blacklisted or debarred by any government, semi-government, or private organization in the past. If found otherwise, my empanelment may be immediately terminated.

I declare that there are no criminal or civil cases pending against me or my firm that may affect my eligibility or integrity as an empanelled agent of UPHDB.

I undertake not to misuse the empanelment status granted by UPHDB. I shall not make any unauthorized claims or misrepresent my relationship with UPHDB in a manner that may mislead the public or potential buyers.
